What to bring to your first appointment
Please come ten minutes early to your first appointment so you’ll have time to fill out our intake forms. You’ll need:
- Your family doctor’s name and phone number
- A list of current medications and existing medical condition
- Your health insurance card and policy booklet if you have one
- A list of allergies and sensitivities

Financial Policy
We require full payment upon the completion of treatment. We accept all forms of payment.
Insurance
Insurance policies vary in the amount of coverage they provide. We’ll do our best to help you understand your insurance coverage, and we can contact your insurer for clarification on coverage for specific treatments. Please note that you are responsible for the cost of any treatments not covered by insurance.
